Report of Death Requirements

Report of Death of a Filipino Citizen Abroad

The death of a Filipino citizen abroad should be reported to and registered with the Philippine Statistics Authority (PSA) through the Philippine Embassy or Consulate having jurisdiction over the locality where the event took place.   In Guam,  the Commonwealth of Northern Marianas Islands (CNMI),  the Republic of Palau,  the Federated States of Micronesia and the Republic of the Marshall Islands,  the Report of Death should be filed at the Philippine Consulate General in Agana, Guam.

 

Basic Requirements for Report of Death

 

  1. Duly-accomplished and signed Report of Death (ROD) form (4 copies)
  2. Information on the deceased, form is attached (4 copies)
  3. Certificate of Death (present original together with 4 copies) issued by the Department of Health or Office of Vital Records or any other appropriate of the state, territory or country where the death took place. 
  4. Copies of itinerary to include full flight details and mode of transport (original plus 4 copies) 
  5. Four (4) copies of valid passport (or identification card) of deceased 
  6. Processing Fee of US$25.00

FOR CREMATED REMAINS WHICH WILL BE BROUGHT TO THE PHILIPPINES (AND WILL BE ISSUED A CONSULAR MORTUARY CERTIFICATE)

 

  1. Certificate of Cremation issued by the Crematory (original plus 4 copies)
  2. Notarized Mortuary Certificate
  3. Burial-Transit Permit (Urn)
  4. Passport or identification card (4 copies) of the person who will travel with the urn 
  5. Fee of US$25.00 for Consular Mortuary Certificate 
  6. Fee of US$ 25.00 for Certificate of No Objection

FOR HUMAN REMAINS WHICH WILL BE BROUGHT TO THE PHILIPPINES (AND WILL BE ISSUED A CONSULAR MORTUARY CERTIFICATE)

 

  1. Notarized certificate (original plus 4 copies) issued by the Mortuary Director that the body was properly embalmed or cremated, and that the casket contains only the body of the deceased.
  2. Certificate of No Contagious Disease issued by health authorities  of the state, territory or country 
  3. Burial transit permit (original plus 4 copies)
  4. Name, address and contact number of the receiving funeral parlor in the Philippines
  5. Fee of US$25.00 for Consular Mortuary Certificate 
  6. Fee of US$ 25.00 for Certificate of No Objection

PLEASE NOTE: If you are submitting your documents by mail, please send the required documents via email for pre-screening. The Consulate will notify you via email whether your documents are in order or if additional documents are required. Once the Consulate confirms that your documents are complete, please mail the original documents along with copies, payment and pre-paid self-addressed envelope. Be sure to include a cover letter/page with complete contact information, including mailing address, contact numbers, the specific consular service you are requesting.
