Report of Birth of Child Born Abroad of Filipino Parent/s
The birth of a Filipino should be reported to and registered with the Philippine Statistics Authority (formerly NSO) through the Philippine Consulate General in Agana, Guam having jurisdiction over the locality where the event took place (i.e. Guam, Commonwealth of Northern Mariana Islands, Republic of Palau, Federated States of Micronesia & Republic of Marshall Islands).
REQUIREMENTS:
- Report of Birth application form, typed or printed legibly in black or blue ink. Forms should be notarized if submitting by mail. – 4 originally signed accomplished form.
- Certificate of Live Birth – 4 photocopies & bring original
- Philippine Statistics Authority (PSA) of formerly National Statistics office (NSO) Marriage Certificate or Report of Marriage of the parents - 4 photocopies & bring original
- PSA/NSO Birth Certificate of father & mother to verify Middle Name of the Child 4 photocopies & bring original
- Current/ Valid Passports of both parents or government-issued identification card (if parent do not have a current/ valid passport) - 4 photocopies & bring original
- Passports of parents (valid at the time of the birth of the child)- 4 photocopies & bring original
- Valid U.S. Permanent Resident Card (Green Card) or a copy of the U.S. Visa of Filipino parent/s - 4 photocopies & bring original
- If applicable, U.S. Naturalization Certificate of parent(s)- 4 photocopies & bring original
- If applicable, PSA/NSO Death Certificate of parent(s)- 4 photocopies & bring original.
- If applicable, dual citizenship documents of parents & child – 4 photocopies & bring original.
- Negative Certification of Birth Record from PSA (CRS Form 1) for applicants whose age is five (5) years old and above.
If Parents are Not Married:
- Affidavit to Use the Surname of Father (AUSF), executed by the mother or the child (with Sworn Attestation of the mother, if child is less than 7 years old) - 4 photocopies & bring original
- Affidavit of Admission of Paternity (AAP) executed by the father or Declaration of Paternity issued by the Vital Statistics Office or Private Handwritten Instrument– 4 photocopies & bring original
- PSA/NSO Certificate of No Marriage of mother - 4 photocopies & bring original
If Parents are Married after Child’s Birth:
- Joint Affidavit of Legitimation by subsequent marriage of parents - 4 photocopies & bring original
- PSA/NSO Marriage Advisory - 4 photocopies & bring original
- Affidavit of Delayed Registration of Birth explaining the reasons for delayed registration, if Report of Birth is filed more than a year after the child is born - 4 photocopies & bring original
- Processing fee: Report of Birth is $25.00, Affidavit of Delayed Registration of Birth is $25.00, AUSF is $25.00, AAP is $25.00 & Certificate of Registration is $25.00 Affidavit of Legitimation is $25.00 (non-refundable), payable in cash, or postal money order, bank draft, certified check or cashier’s check, made payable to “Philippine Consulate General". Personal checks and credit cards are not accepted.
Those filing through the Office of Consular Affairs, all affidavits, including Report of Birth form, are to be notarized in the Philippines.
Note: The Philippine Consulate General assumes no responsibility for any delay or loss in the mail, or while the documents are in the custody of the courier service. The applicant should note the tracking numbers of all envelopes used and submitted.
The Consular Officer may require additional proof or documents from applicant to determine the child’s citizenship, identity, or eligibility for registration of birth under Philippine laws.

mail, please send the required documents via email for pre-screening. The Consulate will notify you via email whether your documents are in order or if additional
documents are required. Once the Consulate confirms that your documents are complete, please mail the original documents along with copies, payment and pre-paid
self-addressed envelope. Be sure to include a cover letter/page with complete contact information, including mailing address, contact numbers, the specific consular service you
are requesting.
